How to file a complaint

You may contact us by phone, FAX, in person, mail or by downloading a complaint form and sending it to us.

We will ask you for the following information:

• Your name, address, phone number and email address
• Your involvement with the complaint
• A brief description of your complaint
• Action(s) you are requesting
• Bureau of Milwaukee Child Welfare (BMCW) staff you have spoken with and what response you have received
